Tulsa, OK – NTSB releases report on fatal Oklahoma plane crash
TULSA, Okla. (AP) Federal investigators say the pilot of a small airplane that crashed in Tulsa and killed five people may have become disoriented by overcast conditions.
Dr. Stephen Lester was flying the Piper PA-32 when it clipped a radio tower's steel wire before slamming into the ground and bursting into flames at Chandler Park on Sept. 5, 2009. Lester; his wife Dana; daughters Laura and Christina, and Dr. Ken Veteto were killed.
The National Transportation Safety Board noted in a factual report that Lester wasn't instrument-certified and took off from Jones Riverside Airport in foggy conditions and when instrument flight rules were in effect.
